Is anyone using a tool to produce end-to-end traceability from requirements to test cases and perhaps defects (if linked to TC or Req)? If so, what are you using? Which 3rd party plugin(s)? Customized development?

What I am looking for is what solutions, if any, are being used to generate full traceability reports? We don't have any plugins implemented yes and before I suggest something I am curious to know how others might have accomplished this. A single plugin? Combo or plugins?
