Form behaviour for submitting data

Hello,

Our business is after a plug-in that enables a form behaviour for submitting data to a database...

Is there a Confluence addin that allows this type of operation to occur?

A form would need to be created with fields that need to be completed for submitting once these feilds (including multi choice options)...

Good idea. But this data should do to the database too. If you are okay with programming, you can use {beanshell} and do all the java programming to submit the data from the run macro to the database.

A different way to look into this is that, if you have JIRA, you can create an "issue collector" and use it as a form in a Confluence page to collect data from users.
