Email Destination Change on Selection

I'm trying to create a request form. However, the issue i'm having is the "destination".

Basically, I would like the form to have the ability to create an email to a specific team AND/OR create a Jira based on what they have selected via dropdown or radial(whichever is easier). However, I have not been able to figure out how to change the "destination" on submit. Can anyone please help me with this?

I believe this is not possible at the moment. I couldn't find any functionality in confluence for this.

I recommend to open a feature request with the exact functionality description you'd like and ask your colleagues to watch and vote to increase the popularity of the feature.

If you can post here the feature URL that would be great since other users can watch and vote as well. :)
