We are currently using Google forms as a manner of intaking ideas from people within the company.
The form itself is conditional. The first question conditionally controls the information requested/displayed to the submitter.
Question 1 - What area can we help with? Web, Mobile, Facility, Staff
Each with have a different set of questions.
There are some required fields within the form and upon submission of the form. A unique ID is created for each submission.
Ie.
Ex-1, Ex-2, etc...
I imagine this would require an app from the marketplace, but I am not having much luck finding a solution to this specific of a use case.

You've posted this question in the Confluence forum, so I'm not sure if you absolutely want to collect the information in Confluence, but a Jira Service Management portal with Forms would allow you to do exactly what you're describing.

@Laura Campbell _Seibert Media_ Is there a Forms app you recommend? I see Proforma is highly ranked but not currently usable with JSM/Jira in the Cloud. Thanks!
