Custom fields using Issue Collector

Hi All, 

I created a Jira request Intake form using Issue Collectors. I was able to create story successfully using the form too.
I have hard coded some of the fields in the form. But the problem I am having is, I see the hard coded fields are getting assigned to their default names - How do I get the same hard coded fields in the story created? Can someone help on this please.

May need some clarification on what you are seeing vs what you expected.
