Create Decision in Decision Log duplicates the page.

I started with the software project template, which added a decision log.  Every time I add a decision via the button on the page, it adds the decision underneath, which works great, but then adds another Decision Log (2) page at the top level of the Site.  

This is a little annoying to have to delete that second page every time we add a decision.  Anyone encountered this or know of I'm doing anything wrong to cause this?

@chris.stout , this is quite odd. I realize this is an old post and I expect/hope that this issue is no longer present. Could you confirm the latest on this and if you found the cause or did it just disappear.

I am having the same problem as Chris! Any idea what could be causing it and how to solve it?
