Anyone know which plugin it is that lets us add comments to the attachments in confluence cloud?

That is delivered functionality that works on a lot of different attachments but not all of them. You will need to add the attachments macro to the page and then view the file through the attachments macro and then you will see the comments feature.
@Brant Schroeder unfortunately it wont work for all of us, only some staff can use it.
Would have been a perfect solution for collaboration, anyone know of another more stable/predictable why of doing the same thing?

We are all licensed users, some people can put comments some, like myself, just get the attached box but no text field.
Seems to be a known issue https://jira.atlassian.com/browse/CONFCLOUD-72035 unfortunately I don't hold a position that lets me see the bug fix.

The know issue you referenced has been marked as fixed so it should not be impacting your users. The users that are having issues what steps are they taking, what does the issue look like (screenshot), what browser are they using? Do they have any browser plugins installed? Have they tried clearing their browsers cache to see if that resolves the problem?

Yes, it is marked fixed but we cannot access the bug fix to see how it was fixed.
We are in firefox, chrome and edge and have cleared the cache and retried.
We are 28,000 strong workforce so we don't have any control over browser details/plugins its a central IT rollout thing.
I think this is an Atlassian support issue and will have the IT team raise a ticket for investigation.

The fix was on the Atlassian side and there would not be any steps for you to resolve the issue other than refreshing the page or clearing cache. Submitting a support issue sounds like a great idea.
