We are reorganising the company and want to change <old name>.atlassian.net/ to <new name>.atlassian.net
Is this possible, if so how?
If not then do I create a new installation the copy what we want from old to new - again, if so how to copy spaces & pages & Apps from one instance to another?

Yes it is possible to change the site URL. Refer to this document:

However, I notice you tagged your post to indicate you are using the Free plan, and it is not possible to change the site URL if all products on the site are on the Free plan, per the above article.
