Automatic Changelog

I am maintaining a changelog for changes in documentation for a particular topic. However, so far I am recording the changes in the changelog manually. Is there a solution which detects changes on a paragraph or heading level and records them automatically?

The feature you describe fits the Page History feature present in Confluence. I believe it will server your objective. Here, take a look:
