How do you insert variables and instruction text to custom templates in December 2019?

I cannot find any of the different ways it's documented. The template item is gone from the toolbar, $ does nothing. How do you, in December of 2019, insert variables and instruction text on a custom template?

Are you using Cloud and are you seeing the variable option as per the below? (hopefully you are  and it's just a bit hidden away! 🤞)
