How to Change Jira IP address and port when migrating servers

How to change IP Address and Port when migrating servers. I have Jira hosted on Windows 2016 server with X:8000 however had to move the server to different subnet.

Now unable to access Jira with same IP and Port, how to update the IP address.

I created a snapshot and launched the same machine in another subnet
