We have recently had two users added to our Atlassian account, and no-one with Admin rights on our account added these users. They happen to be others within our agency -- but our agency has two separate accounts. They should have been added to the other Atlassian account. Have we found a bug?

There are several ways users can be granted access to a site beyond direct invitations from an admin. I would review this document to see if any of these other features are enabled (in particular, the "Approved domains" option: https://confluence.atlassian.com/cloud/specify-how-users-get-site-access-744721636.html
