New org admin cannot access org

Hi,

I added another Org admin to our org. When she attempts to access the admin page she is notified that she needs to create a new org.

She attempted to add an org (with the same name), and is now stuck in the domain creation process (which I already did for our org), and cannot cancel it. 

She can still access all of the applications and appears in the in the users list as a site and org admin.

Any ideas what is causing this strand behavior?
