How to make Jira customer portal form dynamic?

Hello,

I wanted to make our static customer portal form dynamic such that by selecting few value(say from a drop down field), subsequent below fields gets changed.

Right now, my form is static and customer will be shown all the fields that might be required for the type of request they are submitting. 

As per my understanding, scripted fields in ScriptRunner can be help me here but I've very limited knowledge on how to configure.

Please offer your thoughts!
