Force user to enter a comment on workflow transition

Question

When moving an issue from one state to another, I would like to force the user to add a comment to the issue and then allow the transition. How can I do this please? I have already tried the Add Required Field validator.

To summarize you need to

* create a screen with a comment field
* add this screen to the transition
* and make the comment mandatory using a Required Field validator.

@Martyn Hughes-Dowdle you can do this by creating a screen and then associating that screen to the transition
