Card Description Reverted to Original. Where are my updates?

I wrote a description on a card and my team edited it over the course of several weeks. The new description persisted for at least a few weeks, so it's not a matter of forgetting to save it. Today I looked at the card and the description is now the original description that I wrote. What happened? Is there a way to get the updated description back?
