I was thinking about adding Trello to our MS Teams. However not all our members have an active Trello account. Do they need to register in Trello or will they be able to use Trello directly through their current Office 365 account through MS Teams?

Yes, in order to use the Trello integration, user must log in with their Trello credentials in MicroSoft Teams. The Trello app for Microsoft Teams links your Trello teams to those in MicroSoft Teams.
