Staged files cannot be committed

Hello All,

Im really hoping someone can help figure out what is going on.  About a week ago it appears that my commit has stopped working.  

I can create a branch off my master, make a change, and see "uncommitted changes" in my history.  When I click on the "Commit" button in the top right, I can stage my file with no issues, but this is where my problem is...

With all my files staged, I add my comments, and click "Commit" in the bottom right.  I get a "User Information" pop up confirming my user details and click "OK".  At this point, nothing is happening.  My file still shows as staged under my workspace > File Status.  This is where I need help!
