How to customize screen that appears on transition shown on customer portal?

In my service desk workflow, I have two transitions under the Review Status - one to approve and one to reject - both of which are shown on the customer portal so that the customers can approve or reject the resolution provided. 

 There is a default screen that appears after they click on either options which asks for a comment and then adds this comment to the service desk issue. I am not able to customize or find details about that screen.

How do I customise or turn off that screen so that I dont need to opt for an approval process instead.

Thank you for answering.

I had checked all my screens and those associated with the issue operations. None of them have the fields as appearing on this default screen.

@Pauline Monroy, on the workflow scheme .. did you click on the arrow that connects the statuses? It should show details of what screen is involved for that transition.
