how to let customer add multiple options with quantity of each one (like an order form with "Add new" button)?

Can you suggest any plugin? or even a groovy script to be added?

Jira does not have a field type that works that way, and I'm not aware of any apps that can provide one.

The closest I can get you is

• Use a multi-select list - all the entries go into one field, but you can add as many as you like
• Have a look at the apps at https://marketplace.atlassian.com/search?hosting=server&product=jira&query=table%20grid - most of them let you create variable length lists of varying form (although the fields they provide are still single fields)
