Because it needs an email address for you to send email to.
I think I am missing something in your question here though. Could you explain in more detail? "When setting up a custom email" is probably where I am stuck - what are you doing there?

I am setting up service desk and I want all users emailing requests to service desk to use a specific email. i.e servicedesk@test.com
When I click on email requests : I have the option of setting up using email address @Atlassian.net or connect a custom email account. When I select custom email address, I am prompted for a username and password.
Must this be my personal email address? Can this be the servicedesk@test.com? Would this require a username and password as well?

Ok. So, yes, if you use an Atlassian.net address, Atlassian provide the incoming mail service for that (they own the atlassian.net domain, so they can).
If you want it to use another email domain, you are going to have to provide the email inbox for Jira to read. This means providing the mail server, and having a mailbox set up for the address(es) you want to be using, and you have to tell Jira how to log into it to read and process the mail (which is usually the email address and password for access)
