Hi,
I have a basic Request form set up for Jira Service Desk and have enabled a widget for my site. I would like to add few more fields to the form as well as the widget -
However,
I want to be able to add all 3 fields in the form as well as the widget.
Please help!

Thank you for getting in touch with Community!
When using the Widget, we can add system fields for the customer to create the ticket. Some custom fields are not supported, that's why it shows that warning informing that the fields will not be displayed on the widget.
There is a feature request suggesting the implementation of this ability:

Related to the Request participants, it's possible to add only after the ticket is created the same as when creating the ticket using the customer portal, that's why it's not possible to add the field to the widget.
