We occasionally get requests from our users for some pretty common task that often come over e-mail. We'd like to give them a standardized form that they can use to enter these requests so that they're stored centrally and contain all of the relevant data that we need. Rather than building them a Google Doc/form, we want to build them a form that can be submitted into Jira so that we can track the progress of fulfilling the request.
