We are attempting to implement Proforma forms to existing issue/request types.
While I can get the form to appear on the Customer Portal, but it appears below all the activity and comments.
This means that a form that is added at a particular point of the workflow is hidden at the bottom of all the activity.
Further to this, if the form is long when the customer submits the confirmation pop up appears up the top of the display and not seen unless the user scrolls back up.

I suggest you open a support ticket with Atlassian so they can investigate and perhaps eventually provide a fix.

This is unfortunate how the system works.
There are no options to place the from on top.
Is there a reason to add the from later on in the flow, then to just use it from the start on the request type?

We have an initial form that our user fills in through a standard request.
Example being a staff member goes on travel; on their return they are required to fill in a form. Thus allowing the team working with the requests to evaluate and process the final steps.
The users filling in these forms are not service agents, so needs to be displayed in the customer portal.
We were hopeful that the inclusion of Proforma would help us with some use cases. Thus far it is limited due to things like this, and the inability to function with pre-existing third party add-ons that we obtained for missing form display.
