I want in JIRA service desk, when any user makes requests or raise request on behalf of user.
That person details related to Name, ext, floor and direct manger should appear,
how can this be done?

You will need to use an app to accomplish this. The one we use and have been very happy with is User Profiles for Jira https://marketplace.atlassian.com/apps/1212508/user-profiles-for-jira?hosting=server&tab=overview
There are other ones in the marketplace but this one we like the best after evaluating them. I would suggest that you try a couple of them and see which one works best for you before you purchase.

The custom fields that you create for user profiles are locked and populated through the app. I have not tried exposing them in the portal but do however have them added on the View Screen in the service desk. This allows our agents to quickly see that information. They can also hover over the user to get additional information.

as Brant Schroeder already mentioned it is not possible to see the personal details in the customer portal, but in the view screen in the service desk project.

Usually for a field to appear in the Portal it has to have both:
Maybe you could set the respective custom fields to a dummy value (hidden) and then replace it via the User Profiles for Jira App post function during the create transition.
This way the field should be shown in the portal after the ticket is created.
