I just read the article about setting up CSAT surveys and it's between 1 to 5 stars. Is it possible to have it be thumbs up and down instead?

The build in CSAT is not highly configurable (more like ON or OFF).
The only way you have CSAT within Jira is the 1 to 5 star rating. Sadly nothing an be changed about that.
If you really need something else you might need to consider a 3rd party survey provider or an app.
