How do I set up a request form and translate the information from Confluence to a Google sheet?

Hi,

I'm trying to set up a form that colleagues within my organization can use to make requests for training materials. I'd like the request to come in the form of a COnfluence page and then for that request data to be copied over to a Google sheet where I can track it. How do I do 6that?

Just for us to better understand and help you, can you please clarify if you want to use the customer portal as a form or do you want to add a form on Confluence to be copied to Google Sheets? I'm asking because this question was tagged as Jira Service Desk.
