Hi,
I would like to create a customer account so they can log in and raise tickets if need be.
1. How do I issue a new user account
2. How do I know the username, email address and customer portal website?

Welcome to Atlassian Community!!
You can directly invite customers from Jira service desk project which allows customers to set their password from their end.
Go Jira Service Desk Project > Customers > Invite
Also if you want supply customer login details manually then create new user in Jira and don't add any groups to user. Next add the newly created user into Customer role of the service desk project.
