We need to flag certain changes for special review to ensure they do not impact our disaster recovery plan. It appears an automation rule could be used to do this. What field(s) should be scrutinized to detect this type of change? How can a filter be used to detect this type of change? Or would it be better to create a new custom field?

There are many different ways to tackle. You could look for key words in your summary or description, you could put a required custom field to capture on the create screen.
You can either use a filter with a subscription to notify, or you can use the Service desk automation to alert a user or transition the issue to specific status for review.
