Looking to setup a request template that allows a user to check multiple items from a list of options and then create tasks to various technicians based off of what was checked in the template.

You can't quite do this OOB, but with some apps you will be able to do this using Automation. Either you set up an JSD Automation rule based on the value of the option that triggers a transition in your workflow. Then in the workflow transition use a post-function that will create an issue. These apps all have that ability: Jira Miscelleanous Workflow Extension, Jira Suite Utilities or JIra Workflow Toolbox.
Or Automation for JIra by Codebarrel is a powerful app that can be used to create issues when a Service Desk request is created.
