How do I remove a patch file from a Crucible review?

I created a pre-commit review in Crucible. The original patch file I uploaded was incorrect so I uploaded a new patch file. I was able to hide the individual files from the first (incorrect) patch using the "Edit Content in this Review" button on the left toolbar. However the patch file is still visible from areas, such as the review Details page.

Your documentation mentions the "Manage Files Dialog" but it is not clear how to get there in the latest version (3.1). Would you please provide instructions or a screenshot of how to open the Manage Files Dialog so that I can delete the patch file?
