how to insert a comment when uploading file to Advanced Table Attachment Table

I am trying to put comment in the comment column of a Advanced Table Attachment Table after i upload an attachment file. 

can someone please guide what should i do?

After adding attachment i am only getting an "Insert" button option and no option to add comment in the comment column of the table.

Same issue.  Users are not able to add a comment when adding an attachment or attempting to edit the Attachment Table.  Any advice on how to enter text on this field is appreciated.
