Hello there!

Elizabeth, can I ask some details about your environment?

  1. Are you using the Confluence App in both devices (iOS and Android)?
  2. Are you accessing Confluence via the Mobile browser in iOS or Android?
  3. How was the form created?
  4. Has this worked at some point in iOS too?
  5. Are you able to open the form in Destkop (Windows or macOS)?
