my confluence can not create space

Hello,

1. What is your Confluence version? Disregard, you included your instance URL, and I see you are on 6.9.1.
2. Does this occur for all users when they try to create a space?
3. Does this occur in all browsers or only specific ones?
4. When did this last work? 
5. What database version are you using?
6. Please check your Confluence server logs and let us know what error message you see. The message you sent us now is only a warning and doesn't relate directly to the error that is being thrown when you attempt to create a space.
