how to include the created jira issue story number in the email communication sent to the requestor?

Hi I’m newbie and new confi forms aswell :) 

Once the confluence is submitted and once the jira is created in board… and email confirmation will be sent to the requestor… how to include the created jira issue story number in the email communication sent to the requestor ? Please help
