how to add comment when insert file in confluence page

when I insert a file -> select a file from local disk, there is a column -comment.

how to edit this column?

Peter, you can update the Comment by editing the properties of the attached file by clicking the properties link for the attached file.  More information here about 1/2 way down:

Yup, when viewing the page, you'll see your file attachments.  there's a ">" in front of the file name, click it to expand details on the file, then click properties to add comments.
