Why can't a Confluence Admin add a (code block) macro to a page?

Those new to the Atlassian Community have posted less than three times. Give them a warm welcome!

A Confluence Admin (let's call him Tom), being a member of confluence-administrators and confluence-users, can't add a code block, or any other macro, to a page.

At the moment that Tom tried to add the macro to the page, the error "You are not permitted to perform this operation" is encountered.

The group confluence-administrators has all permissions on Global Permissions, and Space Permissions for all spaces.

The example page I'm testing with does not have Restrictions.

Any ideas would be much appreciated, so that Tom can get on with doing his work and have a great day!

In case anyone has this issue in the future, it was due to incorrect commenting within server.xml.
