What is the best way to use forms in Confluence?

Is there a way to capture information in a form that is posted in a page after submission and based in a template?

Using ConfiForm now, it looks really powerful and no too hard to use.

There are plugins for this task. For example, Adaptivist Forms for Confluence addon.
