User forms in Confluence

What is the best way to create a user form in Confluence? I want to design a form to be used by more than 1 team. Some of the fields will be populated by a team 1 while the remaining fields in the form will be populated by team 2. At the end of the day, I want to see all the fields as one form.

I found about 'Easy Form' and created a form using Easy Form Macro. However I am not able to figure out how does the other team member enter the remaining details in the form. Also after I submit a form, am not able to see the form I submitted.

Appreciate if someone can guide me the best way to achieve this...

You should probably contact the vendor of the EasyForms plugin to get support.

We are the vendors of some other plugin for building the forms and workflows  - ConfiForms.

And in our plugin you can make the fields restricted to be filled by "other team" (restrict edits, restrict field's visibility, etc) and a lot more.
