Table of contents link not working

I am trying to add a Table of Contents macro to my confluence page, and it renders in the preview, but when I click "insert" nothing happens on the page.

Could you tell us a little more about "when I click insert nothing happens"?   Do you mean nothing renders where the macro is supposed to be when you visit the page after saving it?  Or you click "insert" and nothing happens at all, the macro definition box stays open and you can't get back to editing?  Or something in between?
