Link External form to Jira

Dear All,

I will like to design a system on JIRA where customer can raise a bug related to our system. In this system I want customer to be able to link to an external form to fill the form. I have designed the form. I want to find out if this is possible and if so how. I have tried everything possible but I could not get to that point.

I also want to find out if is possible to also get the response from the outside form into JIRA such that our developers can have access to it

You can use Jira Rest Api to communicate with Jira from an external form.
