Does a customer need to pay for a license to give us access to their site while we already have confluence licenses?

Confluence license is based on the number of users that have access to a specific site. If you have a license for your site, it doesn't mean that you can access all the other sites without counting as a user.

Yes, licences are per system, not "this person can use the same software in different places".
A licence for an Atlassian system is not given to the user. The licence says "up to X people can have access into this installation"
