Import PDF as a template

I have a PDF that I need filled out from time to time. Is there a way to take the vendors blank PDF form, bring it into Confluence in a way that would allow employees to fill it out and save it, keeping the "template" blank for the next person to use?

Check out the docs on Confluence Page Templates. That feature is exactly intended for this purpose. You can copy and paste the PDF content in to the template to get started, and adjust from there.
