I have an issue with my dates when it gets converted into a jira ticket

Those new to the Atlassian Community have posted less than three times. Give them a warm welcome!
So I built this confiform that creates a request for training then I've formated the dates as dd/MM/yyyy but when it spits out the ticket the info on the description are just random numbers like this please help
