How to pass real-time input to 3rd app/webpage?

I am new to confluence, and I want to add one page with one form:

name: ____

cell phone: ___

submit (this is a button)

Once one user key in the 1st and 2nd information, and click submit, my personal webpage will get this information and do the following action.

Is this doable?

Confluence doesn't come with a way to add forms, but you can do so using a 3rd party add-on.
