How to create a questionnaire

Is it possible to create a questionnaire in my team's Confluence space? I want something that every time someone accesses the page there is a blank questionnaire.

There are several 'Survey' (& poll) add-ons on Marketplace. A couple of them are free. You may want to give them a shot and see which one suits your requirement.
