How do I restrict users to only see older Approved versions of Revised documents?

I want to build a workflow that allows documents to be edited or revised.  When document is being revised, it will show the Approved version to everyone except the person working on the revision.

Is this possible without 3rd party plugins?

No, currently that is not possible to do without an app. I have used Comala Publishing in the past for this scenario together with  Comala Document Control for the review process.

Wow, thank you for the fast response!  

Would it be possible to automate adding a banner when the page goes back into revision?  I'm thinking the banner could link to the most recently approved version, and be removed once the page goes back into an approved state.

That's a good question and looking at the automation for Confluence you would have to use a web request to update the page, and I am not sure if that would leave it in draft mode or if that request would also publish the page. I know automation will support the new live edit which was announce here.

Comala workflow apps (there are three catering for varous scenarios), add their own 'label' to every page indicating the workflow status.

You'll find it among the details such as last updated, 2 min read, 10 people viewed etc.

What @Mikael Sandberg suggests that you'll have two spaces - your working space with the workflow app and restricted access, and the target space for content viewing with more open access.

The Doc Control and Publishing app work together. 

So your page can go from Draft to Review to Rejected to Review to Complete. When it hits Complete, it can be automatically or manually 'Published' to the Target space by Comala Publishing. This automation is built into the apps, no need for Confluence automation.

The idea is that you keep your workflow (the document preparation process) in one space, while your readers will only access approved pages.
