How do I build a drop down list in ConfiForms Field Definition Macro?

I want to be able to have the people who interact with the request to have a quick drop down list to choose from, ie. customer's name and an available list of our customers, instead of people having to type them out.

One of the field types is "dropdown".  Select dropdown, then click the + in the id column.  Each option int he dropdown must have an id and a value.
