As for initiatives in Jira, I am just wondering how the hierarchy is automatically updated in Confluence at the time you change the initiative status in Jira?

Thank you for raising this with us. Issues from Jira are displayed in Confluence using a Jira issues macro. If you have a page in Confluence displaying your data from Advanced Roadmaps, it's likely using one of these macros.
Any hierarchical changes you make to your issues on the Jira side will be stored in a field within Jira. For Advanced Roadmaps, the Parent link field becomes activated, which is where this data is kept.
So, if there's certain information you want to display in Confluence from Jira, you just want to make sure to edit the Jira Issues Macro configuration to display this. You can also use JQL search queries or filters from Jira within this macro to display the information you require. This will be updated real-time when you make changes in Jira.
