Custom create form and workflow in Confleunce and Polling

I have a requirement to create forms in Confluence, however in addition want to have the ability to review the results of the survey/forms and have he ability to vote on the submissions. 

I have found add-ons that have the capabilities for create different forms (Forms for Confluence, ConfiForms) but having the voting component on the results I have not found anything yet. 

Any suggestions ?

Hi @Brian Taylor, we implemented Confiform and we are using the voting component, send email upon creation and creating pages from the same form. I really like the add on and it is very powerful. It will require some learning to start, but the resources available from the vendor are really good.
